The Nevada State Immunization Program is expanding the Pharmacy Partnership Program to now provide residents and staff at Long Term Care Facilities access to COVID-19 vaccines, as well as other routine vaccines, such as those protecting against Hepatitis, Influenza, Pneumococcal, Shingles, and Tdap. There is a dropdown menu to choose vaccines you are interested in having administered at your facility to residents, staff, and family members. In addition, you will be able to request Evusheld, a pre-exposure prophylactic medicine administered intramuscularly by your pharmacy partner that prevents COVID-19 infection for immunocompromised persons and those with a prior adverse reaction to COVID-19 vaccine. By completing this new survey, you will provide us with your contact information so that a participating pharmacy or local health district can contact you. Your facility information will only be given to your pharmacy partner or to provide you with updates on the pharmacy partnership program. This survey is being sent to all Care Facilities in Nevada and should take less than 10 minutes to complete. If you are interested in the Nevada Long Term Care All Vaccine Access Program (LTC 3.0), please fill out this new survey, even if you have already filled out the prior COVID-19 survey. The Nevada State Immunization Program is working to ensure all Nevadans have equal access to recommended vaccines.
